The Network Provider Contract gives OSEEGIB the ability to terminate a Network Provider
without cause upon 30-days notice. All HealthChoice Network Providers have the ability
to terminate the Contract at any time provided a minimum of 30-days notice is given to
OSEEGIB.
Letters of termination from the Network Provider and/or HealthChoice should be sent
certified mail per the terms of the Network Provider Contract. The return receipt will
serve as verification that the information has been received. The actual effective
date of the termination will occur 30-days from the date the termination letter is
received in the Provider Relations Division of HealthChoice.
Please make a reasonable effort to inform all of your HealthChoice patients about
your termination so that they can make informed decisions about future Provider
utilization.
