A dental pre-estimate is an itemization of proposed dental services and the expected
charges before dental services are performed. A pre-estimate is not required, but it
is recommended if the dental treatment plan proposed by the dentist is expected to
cost more than $200, as it will explain how much the member is expected to pay. It
should be identified as a pre-estimate and submitted in the same manner as a standard
paper dental claim.
A dental pre-estimate should not be confused with prior authorization.
