New ClaimLink Site is Available
HealthChoice is proud to introduce an online service from EDS that allows providers to
check the status of medical and dental claims and submit claims electronically, to name
just a few of the great options available day or night.
Security is a primary concern of HealthChoice and therefore, you will need to register a unique username and
password to gain access to your information.
If you used ClaimLink prior to January 1, 2009, you must create a new account and login to
access the site now.
IMPORTANT: You must first register as a provider for each location for each network you are
contracted with. To register, you will need to have the following information available:
- Your ten-digit NPI number or your SSN (Provider ID).
- Your Provider Identification Number (PIN). To find your PIN, access your record(s) in
the provider directory by clicking Provider Search in the menu to the left. You must
register each location for each network you are contracted with.
- A valid claim number (claim ID) issued by either EDS or Fiserv and paid within the last
two years. Do not include the last two numbers after the dash. If the claim was paid by
Fiserv, you will need to add the prefix HC to the claim number.
After you have completed your initial registration, you will be asked to create your own
unique username and password for future use.
Once you have registered as a provider, you can create delegates. For information on how
to create a delegate, click on the link below and scroll down to the FAQs at the bottom
of the Provider Portal home page. The delegate will need to enter the last four digits
of his/her driver license number (DLN) and the delegate code that is assigned after you
register as a provider.
