All benefits and limitations of the HealthChoice Medicare
Supplement plans are governed in all cases by the relevant plan documents,
insurance contracts, handbooks, Rules of the Oklahoma State and Education
Employees Group Insurance Board, and the regulations governing the Medicare
Prescription Drug Benefit, Improvement, and Modernization Act. The Federal
Regulation at 42 C.F.R. § 423 et seq. and the Rules of the Oklahoma
Administrative Code, Title 360, are controlling in all aspects of Plan benefits.
No oral statement of any person shall modify or otherwise affect the benefits,
limitations, or exclusions of any plan.
Your Medicare Prescription Drug Benefit and your rights
and responsibilities are governed by Oklahoma laws and federal laws. The primary
federal law that applies to this document is Title XVIII of the Social Security
Act and the regulations created under the Social Security Act by the Centers for
Medicare and Medicaid Services (CMS). In addition, other federal and state laws
apply.
