To give us your opinion of ClaimLink
ClaimLink is intended to be an easy-to-use online tool designed to give
members access to the most frequently requested claim and benefit information 24
hours a day, seven days a week. Your opinion about its usefulness is very
important to us. Please take a minute to complete a short online survey about
your experience with ClaimLink by logging in with your user name and password
and clicking ClaimLink Survey towards the bottom of the page.
Online Service for HealthChoice Members
HealthChoice members now have access to ClaimLink, an online tool that allows
members to check the status of medical and dental claims, check deductible
status, view and print Explanation of Benefits (EOB) forms, order health and
dental ID cards, and confirm eligibility anytime, day or night.
In order to protect your privacy, first-time visitors must register and provide a
recent health or dental claim number. During registration, you will be asked to
create a User ID and Password. This User ID and Password is all that you will need
to access ClaimLink in the future.
Claim numbers can be found in the upper right-hand section of an Explanation of
Benefits (EOB) form. An EOB form is sent to members after every medical and dental
claim is processed. If you are unable to locate a recent claim number, you can contact
Fiserv Health Harrington at 1-405-499-4920 or 1-800-782-5218 and an Explanation of
Benefits form will be mailed to your address on file at our office. TDD users call
1-405-499-4841 or 1-800-941-2160.
